Lowering finasteride dose may help patients stay on treatment

Clinical takeaway: For men with male pattern hair loss who develop sexual side effects on oral finasteride, consider dose reduction. In this retrospective study, symptoms resolved in 75% of patients who reduced their dose, although resolution was more frequent after treatment discontinuation.
Sexual side effects can prompt patients to stop finasteride, potentially sacrificing an effective treatment for male pattern hair loss. Because lower doses of finasteride can retain hair-growth efficacy, dose reduction may offer a way to improve tolerability while allowing some patients to remain on therapy.
Researchers reviewed records from 5,675 men treated with oral finasteride, identifying 100 who developed sexual side effects and had complete follow-up data. Reduced libido was the most common complaint (83%), followed by erectile dysfunction (35%) and ejaculatory dysfunction (6%). Symptoms developed a mean 42 weeks after treatment initiation.
Among the 100 patients, 48 reduced their finasteride dose, 24 discontinued treatment, and 28 continued at the same dose. Sexual side effects resolved in 75% of those who reduced their dose, compared with 96% of those who discontinued and 50% of those who maintained their original dose. Mean time to symptom resolution was approximately 32 weeks with dose reduction, 20 weeks after discontinuation, and 26 weeks without a dose change.
The 75% resolution rate included 32 men whose symptoms resolved after the first dose reduction and four after a second reduction. The remaining 12 men in the dose-reduction group ultimately discontinued treatment. Five men unable to tolerate the lowest oral dose switched to topical finasteride. None reported sexual side effects during a mean 28-week follow-up.
The authors concluded that “finasteride dose tapering is a practical strategy” for patients who experience sexual side effects but wish to continue treatment.
