Wegovy pill after injections: What happens to weight?

Clinical takeaway: For patients who want to move from an injectable weight-loss medication to an oral option, discuss the daily dosing routine and monitor weight and tolerability after the switch. These early data are encouraging, but they do not show how switching compares with continuing injections.
With oral semaglutide now available for weight management, clinicians need evidence on what happens when patients switch from injectable treatment to a daily pill. In the OCTANE study, adults who switched from injectable semaglutide or tirzepatide to oral semaglutide (Wegovy) lost weight over the next three months, according to findings presented at the European Association for the Study of Diabetes 2026 meeting.
The retrospective analysis used records from a telehealth platform. It included 194 adults with overweight or obesity who remained on oral semaglutide for three months and had weight measurements at both time points. Participants had no history of diabetes or bariatric surgery.
From the time they started the pill, participants lost an average of 4.1% of body weight, or 8.8 lbs. About 41% lost at least 5% of their starting weight. The proportion with a body mass index of 30 or higher fell from 87.1% to 65.5%.
These results apply only to patients who stayed on the pill for three months and reported their weight. Without a group that continued injections, the study cannot show whether switching leads to more or less weight loss than staying on injectable treatment.
For patients considering a switch, the daily routine matters. The tablet must be taken on an empty stomach in the morning with no more than 4 ounces of water, followed by at least 30 minutes before food, other drinks, or oral medications. Discuss whether that routine fits the patient’s schedule.
“What the OCTANE data showed is that a switch to oral semaglutide may be a viable path for the right patient, with real weight loss and lifestyle improvements in the first three months,” said study coauthor Louis J. Aronne, MD, of Weill Cornell Medicine.
